Caldwell Hospice seeks a Full-Time Physician to serve hospice and palliative care patients in all of our service areas including Caldwell, Catawba, Wilkes, Alexander, Burke, Ashe, Avery and Watauga counties. Candidate must have a current physicians license, minimum of two years’ experience; hospice and palliative care experience strongly preferred.
Some duties include:
- Serve as medical consultant to the board of directors, the chief executive officer and the director of nursing services in the implementation of the clinical program reflecting Hospice philosophy, mission, principles of practice and goals
- Serve as a consultant to Hospice staff and volunteers regarding clinical care issues
- Serve as liaison to attending physicians for consultation on medical management and resolution of conflicts over appropriate care to be prescribed and delivered under the professional and financial responsibility of the hospice program
- Participate with the director of nursing services or designee in determining initial and ongoing patient medical eligibility for patient care unit and home care program
- Perform hospital consults at Caldwell Memorial, Watauga Medical Center, Cannon Memorial and Ashe Memorial
- Perform home hospice consults in High Country and Caldwell
- Provide direct physician services to patients in the patient care unit and procedures
- Rotate 24-hour call of physician services and complete all on call duties
- Be available to make house calls as determined necessary by primary team
- Make and document all face to face visits for High Country hospice patients
- Complete Primary Plan of Care for assigned patients
- Complete death certificates as appropriate
- Complete initial certification and recertification of patient’s terminal condition in a timely manner to meet the Hospice Medicare Conditions of Participation
- Supervise and back up
- Assure all license, certifications, continuing education information and any other required documentation are submitted to human resources and kept current
- Maintain up-to-date documentation required and specified in Hospice policy and procedure
- Complete physician log on a timely basis and submit to supervisor

Full-Time On Call Registered Nurse—Caldwell
Caldwell Hospice is seeking a full-time On Call Registered Nurse to rotate call responsibility with our on-call team, working three days, off four. Candidates must have a current NC license, CPR certification, two or more years of experience, great communication skills, and a positive attitude.
DUTIES AND RESPONSIBILITIES:
- Ability to work a flexible schedule
- Adhere to strict confidentiality
- Be available by telephone or cellular telephone and respond within 15 minutes
- Communicate with on-call team members and patient care unit staff on regular basis
- Evaluate the physical, financial, psychological, spiritual, and pre-bereavement needs of the patient/family in coordination with interdisciplinary team
- Assist in assessing patient appropriateness for services
- If patient is not appropriate, recommend other services available, with referrals to other community agencies, as appropriate
- Educate patient and family about Hospice services
- Coordinate the development of the plan of care with goals defined by the patient/family and the hospice team updating as changes occur
- Coordinate medication administration, education and assure accuracy
- Teach the primary caregiver, sitter, and home health aide by role model, repetition, and reinforcement
- Provide ongoing supervision along with assessment of continuing needs for patient/family teaching
- Instruct caregiver in emergency response and anticipate change
- Make regular visits as ordered by the physician and as needed
- Provide the patient/family with information concerning the disease process, the regimen of treatment process, and alternative methods of care
- Respond to questions honestly and realistically
- Help the patient/family make informed choices
- Provide emotional support
- Support individual coping mechanisms
- Provide services at the time of death
- Assist with bereavement follow-up attending receivings and funeral services as appropriate
- Evaluate the effectiveness of nursing interventions on a regular basis and whenever changes occur in the patient/family situation
- Communicate with physician as needed and required by regulations
- Maintain current knowledge and competency in providing care to terminally ill patients and their families
- Assist with nursing care on patient care unit as directed by supervisor

- Adhere to strict confidentiality
