Contributions

Caldwell Hospice and Palliative Care is a private, not-for-profit community organization, providing physical, emotional, and spiritual care to patients with a life-limiting illness and their family members. A team of professionals, specializing in end-of-life care and including physicians, medical social workers, nurses, certified nursing assistants, chaplains, and specially trained volunteers, cares for patients in their own homes, skilled nursing facilities, rest homes or hospitals. Grief-support services are provided to Hospice patients and their family members and anyone in the community needing these services. CHPC also provides care to patients with life-limiting illnesses and seeking curative treatment through the Transitions program and the Caldwell Partnership for Palliative Care. Education on end-of-life issues is provided to the community through The Caldwell Partnership for End-of-Life Care. These programs are supported entirely by generous contributions received from our community.
